An official of an organization who completes and signs an application accepts complete responsibility for all of the cards issued to the organization and for all materials borrowed on the Organization Cards. |

The authorized person(s) will sign within the signature space of the card(s) issued. |

The card(s) can be used by anyone that the authorized person designates to use the card. This can include a messenger service, employee or volunteer. The authorized person or designee must have the card to borrow materials. |

It is the responsibility of the official who signs the application to notify the Hennepin County Library's Integrated Library System Section if the card(s) is misplaced or stolen. A new library card will be issued and the original card number will be listed as invalid. The organization is responsible for all unreturned materials checked out to the organization up to the date the loss was reported. |

All regular library circulation policies and procedures apply to the use of the Organization Cards. |

Remote access to HCL subscription databases is intended for individual use with a personal library card. Organization library cards cannot be used to access these databases remotely. |

| 7. |
Organization cards are valid for one year. A one-year expiration date is entered at the time of registration. |
| |
| 8. |
The application for an Organization Card is to be completed online or mailed to Hennepin County Library's Integrated Library System Section (address on the application form). The library will mail the library card(s) to the organization. The application can be sent by staff to IT/ILS via the interdepartmental mail. No material may be checked out without a valid, properly signed card. |

If the name of the official accepting responsibility changes, a new application must be completed. |
